Here is how it works:

1) Enroll at the Caregiver's Marketplace site
2) Print a Cash Back request form for 2008
3) Gather your 2008 receipts - you must have 5 qualifying items to send in a form - items do not have to be 5 of the same thing.
4) Complete the Cash Back form and get it postmarked by Sat. January 31st!
5) 4-6 weeks later - get a check in the mail !
